1. Overview
InkForm Pro provides digital documentation tools for tattoo studios, piercing studios, PMU artists, and related body art businesses. It helps studios collect intake information, consent forms, electronic signatures, ID images where enabled, photo and video releases, aftercare acknowledgments, QR or kiosk submissions, and related client records.
InkForm Pro is a documentation and workflow tool. Studios are responsible for reviewing their consent language, privacy and recordkeeping obligations, and local legal requirements.
2. Information we collect
We may collect studio account information, including names, business names, email addresses, phone numbers, login credentials, studio settings, branding information, staff names, roles, permissions, and billing information where applicable.
Client forms may include names, dates of birth, phone numbers, email addresses, addresses, appointment or service details, procedure details, health-related intake responses, acknowledgments, parent or guardian information, electronic signatures, submission timestamps, uploaded ID images where enabled, photo or video release preferences, and aftercare acknowledgments.
For public QR or kiosk forms, we may collect submission details, access or submission times, device and browser information, IP address, and usage logs needed to operate and secure the service. We may also collect browser and device type, operating system, pages and features used, login activity, error logs, and security information.
3. How we use information
- Provide, operate, secure, and improve InkForm Pro.
- Create and manage studio accounts and permissions.
- Generate, save, search, export, and print client records.
- Enable QR or kiosk intake and electronic signatures.
- Create PDFs or downloadable records.
- Send account, security, billing, support, and product messages.
- Detect abuse or unauthorized access and comply with legal obligations.
4. Studio responsibility for client data
Studios are responsible for deciding what information they collect and how long they retain it, using appropriate consent language, following applicable laws, reviewing forms with an attorney where necessary, obtaining required guardian consent, protecting client records, responding to privacy requests, and deciding whether ID capture is necessary. InkForm Pro does not provide legal advice.
5. Electronic signatures and records
InkForm Pro may allow users and clients to sign forms electronically. Electronic signatures, checkboxes, initials, typed names, drawn signatures, timestamps, and submitted records may be used as electronic records. Studios are responsible for confirming that their use is appropriate for their location and requirements.
6. How we share information
We do not sell client form submissions. We may share information with the studio that collected it, its authorized staff, service providers that operate InkForm Pro, payment processors, hosting, database, security, analytics, and email providers, legal authorities when required, professional advisors, or a successor in a sale, merger, acquisition, or business transfer.
7. Data storage and security
We use reasonable administrative, technical, and organizational safeguards designed to protect information. No online service, database, or transmission method is completely secure. Studios should use strong passwords, limit staff access, and maintain appropriate internal security practices.
8. ID images and sensitive information
Studios may choose to collect ID images or health-related intake responses. Studios should collect sensitive information only when necessary and follow applicable storage, access, and retention laws.
InkForm Pro is not intended as a medical records system. Unless separately agreed in a written Business Associate Agreement, InkForm Pro is not offered as a HIPAA-covered service.
9. Cookies and tracking technologies
InkForm Pro may use cookies, local or session storage, analytics tools, and similar technologies to keep users logged in, remember preferences, improve performance, analyze usage, protect against fraud, and support marketing or advertising if enabled.
10. Data retention
We retain information as needed to provide the service, comply with legal obligations, resolve disputes, enforce agreements, maintain security, and support studio recordkeeping. Studios decide how long they are required or allowed to retain client records. Deleted information may remain in backups or logs for a limited period before deletion through normal retention processes.
11. Client privacy requests
Clients should first contact the studio that collected their information. If a client contacts InkForm Pro directly about a form submission, we may refer the request to the studio that collected it.
12. California privacy rights
California residents may have rights to know, access, correct, delete, or limit certain uses of personal information depending on the circumstances. InkForm Pro does not sell client form submissions. We may need to verify identity before responding to a request.
13. Children and minors
InkForm Pro is intended for studios and their clients. Guardian consent forms may be completed where appropriate. Studios are responsible for verifying guardian authority and following laws for services involving minors. InkForm Pro is not intended for children under 13 to create accounts or use independently.
14. Third-party links and services
InkForm Pro may link to or integrate with third-party services. We are not responsible for their privacy practices, security, or content.
15. International users
InkForm Pro is operated from the United States. Information may be processed and stored in the United States or other locations where service providers operate.
16. Changes to this policy
We may update this Privacy Policy from time to time. Updated versions will be posted with a new effective date.
